Services Includes by our Doctors:
Medication Management Reviews (approx. 60 minutes)
- This review is a joint effort between our primary care doctor and your suggested community pharmacy to help you get the most out of your medications and avoid any possible side effects or other problems.
Health Check for people aged 75 and older (annual) (approx. 60 – 90 minutes)
- This check is for people who are 75 years old or older. It is a structured way to find health problems and conditions that could be prevented or could be helped by measures to improve health and quality of life. It gives you a chance to talk to your doctor or nurse about any problems that are hurting your health and well-being as a whole.
Health Check for Residents of Residential Aged Care Facilities
- A systematic strategy to detect health problems that can be avoided or treated with treatments that increase health and quality of life is provided by this examination, making it useful for long-term residents of an assisted living or nursing home. The resident's pain management, foot care, skin integrity, continence, hearing, vision, and oral health, as well as their capacity to do ADLs and avoid falls, are all assessed as part of this evaluation.
Coordinated Veterans’ Care (CVC) Program (approx. 60 – 90 minutes)
- Patients with congestive heart failure, coronary artery disease, chronic obstructive pulmonary disease, diabetes, or pneumonia who are living in the community and at high risk for unnecessary hospitalisations are the focus of the CVC Programme. Our professional independent doctors will work together a lot to teach you about your health, help you deal with your conditions, and make sure all the services you get are coordinated. Your care plan, which includes things like your medications, future appointments, and health goals, will be written in a way that is easy for you to understand and will be reviewed with you regularly.
